Most change order templates capture the money and stop there. The two that actually prevent the argument are the schedule impact and the authorisation line. If a change adds four days and nobody wrote that down, the completion date does not move in the client's head, and you are late on your own job. If the work started before anything was signed, the conversation about whether it was ever approved happens after you have already paid for the labour.
This form prints both. It is not legal advice and it is not a substitute for reading your own contract, which may set out its own change order procedure that takes precedence. What it is, is better than the invoice line item that most changes actually get recorded as.

A signed change order is generally treated as an amendment to your contract, but this is a form, not legal advice. Your existing contract may specify its own change order procedure, notice period or required signatures, and that procedure takes precedence. Have a lawyer review the terms block if the amounts are significant.
